How it Works
- PLEASE NOTE - This form can be used to extend medical certificates. You must have a consultation or telephone consultation to receive an initial medical certificate.
- If you need to extend your cert, you can submit this form and we will contact you if you need to be reviewed by a GP.
- Medical certificate extensions cost €10.
- You will receive an email receipt and payment receipt.
- All payments are processed securely by Stripe, and no credit card information is stored by the surgery.
